Permanent & Transitional Housing Services |

>> Transitional Housing Program – 12 month residential program designed specifically for individuals who are homeless and diagnosed with a psychiatric illness. Residents receive comprehensive support services addressing the needs of the total person in order to prepare individuals to live independently in the community. Services include career training, daily living skills training, supportive counseling, medication monitoring, financial planning, transportation, recreation and housing location assistance.
Franklin House – is a 6 month residential program serving homeless single mothers and their children. The program serves the entire family and is staffed 24 hours a day. The goal of the program is to foster effective parenting, healthy child development and family unity while preparing the family to transition into permanent housing. Comprehensive support services include parenting and nutritional workshops, financial planning, welfare-to-work assistance, supportive counseling, transportation and housing location assistance. |

>> Permanent Supportive Housing – This program provides housing that is both permanent and affordable. Through trained support specialists, we also provide flexible services based on the needs of the individual. Services may include case management, training in daily living skills, employment training, transportation, recreation and supportive counseling. Individuals may choose the services that they require. Housing is not based on the acceptance of these services.
The Franklin House Family Community Support Program – is a post-graduate case management program addressing the recidivism of homeless families – families attempting to transition from welfare to work. This program was developed in response to the overwhelming need to maintain a follow-up program with trained staff to assess family's ongoing problems and supply appropriate services that are essential. The goal of the Franklin House Family Community Support Program is to phase in and foster utilization of natural supports and linkages within the community that would ensure permanent stability for families. |
