>> Clinical case management program designed to service graduates of the Transitional Housing Program as well as other individuals who are consumers of mental health services referred by other community sources. The program’s overall goal is to provide case management support to individuals living independently that need assistance in maintaining their housing as well as to provide linkage to community resources and entitlement programs that have a direct impact on preventing crisis and repeat hospitalization. Services include assistance with daily living skills, financial planning, medication monitoring, supportive counseling, career planning, transportation and recreation.
