Clinical case management program designed to service graduates of
the Transitional Housing Program as well as other individuals who
are consumers of mental health services referred by other community
sources. The program’s overall goal is to provide case management
support to individuals living independently that need assistance
in maintaining their housing as well as to provide linkage to community
resources and entitlement programs that have a direct impact on
preventing crisis and repeat hospitalization. Services include assistance
with daily living skills, financial planning, medication monitoring,
supportive counseling, career planning, transportation and recreation.

School-Based Program - case
management implemented in schools to provide support, educational
counseling and direction to at-risk teens. Professional parenting
programs are provided by staff to pregnant or single teen parents. |
